The crash occurred at around 10:50 p.m. and involved multiple vehicles, prompting a major emergency response from police, ambulance crews, and firefighters. The collision caused significant disruption to the motorway, with long tailbacks reported into the following morning as lanes remained closed for investigation and recovery work.

West Midlands Ambulance Service confirmed that despite the efforts of paramedics at the scene, the woman was pronounced dead at the location of the crash. A man involved in the incident sustained serious injuries and was rushed to University Hospital Coventry and Warwickshire under emergency conditions for urgent treatment.

Two additional men were also assessed by paramedics at the scene and were found to have minor injuries. They did not require hospital admission and were discharged on site after receiving treatment.

Officers remained at the scene for several hours overnight, carrying out forensic examinations and working to determine how the crash occurred. Drivers were advised to avoid the area as lane closures continued into the morning rush hour.

The M40 remains one of the UK’s busiest motorways, and serious collisions on the route often lead to extended delays while emergency crews manage clear-up and investigative procedures.
